This is a task with combined types of activities

1. Create a Web application based on PHP while using CouchDB as the data source. Your simple application should include:
   • Public profile list of users in the address book
   • Individual profile page
   • A page that can be used to create new profiles (All the stuff of a profile should be stored under the database like profile picture, etc).
2. Style above mentioned pages using Twitter Bootstrap (Additionally you may use icon themes like "Font Awesome") while maintaining a nice & attractive UI design.
3. Host your application under OpenShift.
